The story follows the character Ok-ju, portrayed by Jeon Jong-seo, seeking vengeance against a sex offender who pushed her best friend, leading to a tragic outcome. Lee Chung-hyun expressed his preference for female perspectives in his work, finding them rich in potential for various plot twists and elements. The film aims to shed light on the issue of sexual exploitation against women, inspired by real-life incidents in Korea.

While ballerinas in the film symbolize femininity, Director Lee emphasized their strenuous profession, reflecting the duality of appearing elegant while performing technically challenging and intense movements. "Ballerina" has gained significant traction on Netflix, reaching No. 1 shortly after release and receiving positive feedback from international viewers.

The film's music director, singer and producer Gray, collaborated with Netflix for the film's scores, resulting in a stylish and innovative musical accompaniment. Although a sequel was not initially considered during the film's production, the idea has garnered attention, prompting Director Lee to contemplate potential storylines. "Ballerina" was showcased at the 28th Busan International Film Festival, providing a platform for direct interaction with the audience and underlining the unique experience of watching films in a theater.
